Please complete all fields below to Enrol in your CCHM Program. Once we have received your application, you will be contacted by telephone. You may submit your application fee by Visa, Mastercard, Cash, Cheque or Money Order within 2 weeks of your enrolment (note that there is no application fee for the Post Graduate Program). You understand that you will be required to submit your $85.00 application fee within two weeks of submitting this form, and that you will also be required to submit your Life Sketch, Essay, Letters of Reference as well as your original transcripts as required.
